In countries with a high tuberculosis (TB) burden, such as Indonesia, private practitioners play a critical role in managing initial visits of presumptive TB patients. Despite this, many private practitioners often do not follow national TB guidelines, complicating disease control. This study explores barriers affecting Indonesian private practitioners’ adherence to TB management protocols through in-depth interviews and focus group discussions with practitioners and stakeholders.

The findings reveal multiple challenges including inconsistencies between national and international guidelines, complex referral and financing systems, limited access to diagnostic tools and medications, patient preferences, and distrust between practitioners and local governments. These factors collectively hinder optimal clinical decision-making within TB management. DOI: 10.1136/bmjgh-2024-015261
